Impacto de los probióticos sobre el VO₂max en adultos sanos y deportistas: Una revisión sistemática.

La microbiota intestinal desempeña un papel relevante en procesos fisiológicos relacionados con el metabolismo energético, la función inmunitaria y la respuesta al ejercicio físico. En relación con esto, se ha propuesto el uso de los probióticos como posible herramienta para mejorar el rendimiento físico. El objetivo de esta revisión sistemática fue analizar la evidencia científica disponible sobre el efecto de la suplementación probiótica en la capacidad cardiorrespiratoria máxima, medida a través del consumo máximo de oxígeno (VO2max), en adultos sanos y deportistas. La presente revisión se llevó a cabo siguiendo las directrices PRISMA 2020, efectuando una búsqueda sistemática en las bases de datos PubMed, Scopus, Web of Science y ProQuest. Finalmente, se seleccionaron seis ensayos clínicos controlados aleatorizados a doble ciego (N = 274 participantes totales; evaluando futbolistas, ciclistas, corredores, triatletas, jugadores de bádminton y adultos sanos) publicados entre 2020 y 2024. Las intervenciones evaluadas emplearon protocolos monocepa o multicepa, con dosis entre y y periodos de tratamiento de 4 a 16 semanas. La calidad metodológica según la herramienta Joanna Briggs Institute (JBI) reflejó un riesgo de sesgo bajo a moderado. Cinco de los seis estudios observaron mejoras significativas en el VO2max en los grupos suplementados con probióticos con respecto a los valores basales. Sin embargo, estos resultados procedían de comparaciones intragrupo, sin diferencias sigificativas frente al grupo placebo. Asimismo, se constató una marcada infrarrepresentación femenina (73,7% de varones en estudios con datos por sexo), sumando una elevada heterogeneidad en las poblaciones (nivel de entrenamiento) e intervenciones estudiadas (diversidad de cepas y dosificación). En conclusión, aunque la evidencia disponible sugiere una posible asociación entre los probióticos y la mejora del VO2max, existen limitaciones metodológicas y ausencia de superioridad demostrada frente al placebo que impiden establecer una relación causal.
